It is our desire to provide the finest dental care available for your child in an atmosphere of congeniality and mutual trust. Please feel free to review our guidelines. If you have any additional questions or concerns please contact us!
Patients are seen by appointment only. Please call our office to arrange an appointment that coordinates both our schedules. We value your time and make every effort to maintain an on time schedule. If you cannot keep an appointment, we would appreciate a 48 notice as much as possible. If necessary please leave a message (if calling after hours) so we can offer that time to another patient.
AFTER SCHOOL APPOINTMENTS -
To accommodate the maximum number of after school patient visits, we advise you to schedule your appointments as soon as possible as they do fill up quickly. We will try to give as many patients after school appointments as possible, but if school time appointments are necessary, we will gladly provide verification of the dental visit for your school's attendance office. It is state law that children be excused from school for medical and dental appointments.
If you child should suffer a traumatic injury, please call the office immediately. If your child is having an afterhours emergency you will be able to attain the number of the on call doctor via our answering service.
FINANCIAL INFORMATION -
We are committed to providing you with the best possible care. We will gladly create a treatment plan for your family if any follow up visits are needed. You are responsible for the timely payment of your account. Your clear understanding of our financial arrangements is important to our professional relationship.
If you have insurance, we will help you receive the maximum benefits available under your policy by submitting forms promptly.
Insurance policies are contracts between you and your insurance company. We file insurance claims as a courtesy to our patients. We cannot become involved in any disputes between you and your insurance company regarding deductibles, co-payments, covered charges, secondary insurance, etc. We can only supply factual information.
If you do not have insurance coverage, full payment is expected at the time of the appointment. Alternate payment plans must be made prior to the appointment. We do accept cash, check and all major credit cards as forms of payments.
Due to the current social phenomenon of divided families, we have found it necessary to establish some guidelines regarding children who find themselves in this situation.
You can understand that we cannot be caught in the middle of responsibility disagreements regarding appointments or financial responsibility. Therefore, it is hereafter our office policy as the custodial parent (the parent with whom the child resides) will be considered responsible for the child's dental treatment. In the case of divided custody, the parent that brings the child in for the first appointment will be considered the responsible parent. We will deal directly with the custodial parent in all matters concerning the child's treatment.
ATTENTION ATHLETES -
If you are participating in sports where accidental contact is likely, we recommend a mouth guard to be worm during games and practices. You can pick up a kit at a sporting goods store or over the counter at a drugstore. We can help you fit the mouth guard if needed or can even fabricate a custom fitting mouth guard, as well.
HAND HELD DEVICES -
As a courtesy to our patients and staff, we request that you please silence all devices prior to entering the office. If you need to take any phone calls or use these devices, please do so in the waiting room.
